Hours and Benefits: Full-time, 40 hours/week, $25/hour. Benefits include medical and dental insurance, vacation, holiday, and sick pay.


Organizational/Program Background

Central City Neighborhood Partners (CCNP) was established in 1994 to address socio-economic conditions impacting low-income families in the Central City, Westlake, and Pico-Union communities. CCNP collaborates with partners to create pathways to financial security and economic opportunity through the FamilySource Center (FSC) program and other initiatives.


Position Overview

The Supervising Community Health Navigator (CHN) is responsible for overseeing the daily activities of Community Health Navigators (CHNs), ensuring the implementation and refinement of the CHW SON project. Key responsibilities include:

  1. Supervision and Coordination
    • Provide daily supervision and support to CHNs.
    • Assign outreach tasks and ensure adherence to Public Health recommended scripts.
  2. Service Quality Monitoring
    • Ensure culturally and linguistically appropriate services are provided.
    • Monitor and enhance referral workflows for improved efficiency.
  3. Partnership Development
    • Build partnerships with local Community-Based Organizations (CBOs) to strengthen the resource network.
  4. Reporting and Documentation
    • Prepare and submit various reports related to project activities and outcomes.
  5. Training and Capacity Building
    • Provide training on partnership development and system navigation skills.
  6. Community Engagement
    • Identify outreach strategies that resonate with the target population.
    • Establish feedback loops with community members to improve services.
  7. Additional Duties
    • Maintain documentation of families’ program participation and client files.
    • Perform other tasks as assigned by leadership.

Qualifications and Experience

  • Bachelor’s degree in Public Health, Social Services, or a related field; or five years of experience in a similar role.
  • Two years of experience as a Senior Community Health Worker or Lead System Navigator preferred.
  • Strong interpersonal, verbal, and written communication skills, with proficiency in English and Spanish.
  • Familiarity with Microsoft Office applications and database management.

Other Requirements

  • Successful completion of pre-employment screening, including background check.
  • TB Certificate and full COVID-19 vaccination required.
  • Valid California driver’s license and reliable transportation.
  • Ability to lift up to 25 pounds and perform physical tasks as necessary.
