Community Health Navigator (Promotora)

Organizational/Program Background:

Central City Neighborhood Partners (CCNP) was established in 1994 to address socio-economic conditions impacting low-income families in the Central City, Westlake, and Pico-Union communities. CCNP collaborates with partners to create pathways to financial security and economic opportunity through the FamilySource Center (FSC) program and other initiatives.

Position: Community Health Navigator (Promotora)
Hourly Rate: $18.00
Full-Time: 40 hours per week

Position Overview:

The Community Health Navigator is responsible for conducting community engagement, outreach, and navigation to provide direct referrals to services for customers most impacted by the pandemic. The goal is to reduce health disparities by addressing social determinants of health, offering early intervention, and promoting health education. This position will also provide program and administrative support to ensure effective, efficient, and consistent implementation of the program in alignment with grant requirements and the mission of CCNP.

Qualifications and Experience:

  • High School Diploma or two (2) years of promotora experience.
  • Six (6) months of experience working with the public or community groups.
  • Knowledge and experience in community health and social services.
  • Strong listening, communication, and interpersonal skills.
  • Creative problem-solving ability and initiative.
  • Excellent organizational and administrative skills.
  • Cultural competency and flexibility in working with diverse populations.
  • Proficiency in English; Spanish proficiency is a plus.
  • Familiarity with Microsoft Office applications and data management systems.

Other Requirements:

  • Full COVID-19 vaccination.
  • Possess a TB Certificate and pass a Live Scan background check.
  • Reliable transportation and a valid California driver’s license.
  • Ability to lift up to 25 pounds and perform physical tasks as necessary.
