Organizational/Program Background

Central City Neighborhood Partners (CCNP) was established in 1994 to address socio-economic conditions impacting low-income families in the Central City, Westlake, and Pico-Union communities. CCNP collaborates with partners to create pathways to financial security and economic opportunity through the FamilySource Center (FSC) program and other initiatives.


Job Title: Supervising  Navigator

Salary: $25.00 per hour

Hours: Full-time, 40 hours per week


Qualifications:

  • Education: Bachelor’s Degree in Public Health, Social Services, or a related field.
  • Experience: Minimum of two (2) years as a Community Health Worker or System Navigator, or five (5) years of experience in a similar role.
  • Certifications: Full COVID-19 Vaccination, TB Certificate, and completion of Live Scan background check.

Position Overview:

The Supervising Community Health Navigator (CHN) plays a critical role in overseeing the daily activities of Community Health Navigators. Responsibilities include:

  1. Supervision and Coordination:
    • Provide daily supervision and support to CHNs.
    • Relay updates and communicate relevant information to the team.
    • Coordinate and assign outreach tasks among team members.
    • Ensure adherence to Public Health’s recommended scripts during outreach efforts.
  2. Service Quality Monitoring:
    • Ensure that project staff deliver culturally and linguistically appropriate services.
    • Monitor and refine referral workflows to enhance efficiency and effectiveness in service delivery.
  3. Partnership Development:
    • Identify and establish partnerships with local Community-Based Organizations (CBOs) to strengthen the resource network available to the community.
  4. Reporting and Documentation:
    • Prepare a variety of reports to track project outcomes and impact.
    • Document team activities and monitor progress toward project goals.
  5. Additional Responsibilities:
    • Engage in team meetings to discuss challenges, share solutions, and foster collaboration.
    • Participate in training and professional development opportunities as needed.
